To become an official member, you need to fill in the form and pass to our committees.
You may request the form thru online from Carsen or committees.
The fees to become official member is RM50 (whole life).
You may pay by cash or bank in to Carsen.
This is including 1 S size MIX Team sticker + 1 L size MIX Team sticker + 1 MIX Team code number sticker.
Please return the form after you fill in, and please follow our procedure to stick up MIX Team stickers.
Now, welcome to our big family
